TEFAP, or The Emergency Food Assistance Program, is a valuable resource for many families, allowing them to access free, USDA-purchased food through food banks and pantries. TEFAP outlets must meet certain criteria and be approved by the food bank to participate in this program. Please contact your Network Service Manager if interested in learning more.

Income Guidelines Poster

The TEFAP Income Eligibility chart breaks down the relationship between household size and income requirements. Income guidelines change annually upon release of new federal guidelines.

Written Notice of Beneficiary Rights

This notice informs recipients that they cannot be discriminated against based on religion, required to participate in religious activities, or denied services. TEFAP outlets must provide this notice before enrollment and ensure recipients know how to report violations.

Proxy Form

TEFAP proxy forms allow a recipient neighbor to authorize another person to pick up food from a TEFAP outlet on the recipient’s behalf.

Individual Eligibility Certificate

TEFAP outlets may choose to utilize either the Individual Eligibility Certificate or the Aggregate Eligibility Certificate. By providing information on this form, neighbors verbally attest that they meet the current Income Guidelines. No signature is required.

Aggregate Eligibility Certificate

TEFAP outlets may choose to utilize either the Individual Eligibility Certificate or the Aggregate Eligibility Certificate. By providing information on this form, neighbors verbally attest that they meet the current Income Guidelines. No signature is required.

TEFAP outlets are required to have this document on hand as a tool to assist in identifying what language a neighbor is most comfortable communicating in.

Civil Rights

TEFAP outlets are required to post the USDA AJFA poster where visible to TEFAP recipients. Contact your NSM if you need a new poster.

Volunteer Civil Rights Form

Each volunteer must complete civil rights training at least once per year. TEFAP outlets may choose whether they’d like to use the training form or IDOH video. Confirmation of training completion must be kept on file at the outlet for a minimum of four years.

Civil Rights Training Log

In order to confirm that TEFAP outlets are completing civil rights training at least once per year, TEFAP outlets are asked to complete a training log and send to their Network Service Manager. (Please note that this log does not have to be all-encompassing of every volunteer and serves as a confirmation mechanism. It is recommended that the log include all regularly recurring volunteers.)
